I don’t know how V3 is, but if it lists the note lengths like V2 (…, Whole, Breve, Longa), then after Longa should be the Maxima note. It looks pretty much like a Longa note.

Can you tell me which version do you have when you visit this page? I get a glitch too in firefox, but it isn’t too bad as it’s gradually fadeout, whereas the one in chrome was pretty bad with a high volume but since the latest update, in chrome, it seems to just not do it at all anymore.
Also how do you trigger it? For me I just open up a couple of pages and press play on a bunch of them while one song was already playing.

Can you update it by going to this page? Makes sure to uncheck the box so you don’t get a crap toolbar. It was a pretty recent update to the flash player that seems to fix the glitch.
I retested it using the new version of Adobe and I still managed to get the EarRape glitch with IE. I opened 6 tabs all playing different songs while engaging in a conversation with Cleverbot. At first, the EarRape glitch happened pretty much every 2 seconds. But after a while, it became rare. And I also didn’t get the EarRape glitch as I was closing out tabs. When clicking on another page while the players are playing, I still did not get the EarRape glitch.
Conclusion:
EarRape occurred when they were all loading at the same time.
I have uncovered an odd glitch with the new player. The length of this song is “-9: -8: -5”. Though that isn’t the real problem. It seems that the song never has a measures 200+. It’ seems to be only repeating the first 199 measures (You’ll notice this if you start skipping through the song). Also, sometimes the new player will skip back to the beginning when you are clicking all over the place.

I had a Twitch.tv stream open and running at the same time. I opened a Google+ page and it happened. I closed that page, turned to the Twitch tab and it happened again.

Sometimes it doesn’t ear rape, it fades out.
That even happens to me, i hate it when it happens.
That usually happens when the player’s volume is closer to 0%. It’s more likely to cause ear rape when the volume is closer to 200%.
Then I’ve probably caused the most earrapes… Or Verbist, whatever comes first.
